BASIC SEARCHES

Several Basic Searches are available by clicking the Basic Searches tab and selecting from a pick list of searches. Basic Searches are common searches that many users routinely perform (for additional search capability, use Advanced Searches).

Item Number - Each NMED record is assigned a unique Item Number.

Field Name Help Text (Displayed when clicking on field label)
Item Number The unique six digit item number. The first two digits represent the year in which the event was entered into NMED. The last four digits are the order in which the event was entered in that year. For example, the first event entered in 2004 will have an item number of 040001. To search for multiple item numbers (up to 15), seperate each with a comma or space.

Lost/Abandoned/Stolen Material - This search returns only NRC reportable events involving AEA material with event dates greater than 1/1/1996. If an event involves both AEA and Non-AEA material, the Non-AEA material will be included in the source count. However, if an IAEA category is selected, the source count is limited to sources with the selected IAEA category. Well logging sources abandoned downhole are not included in this search. Note that NMED implemented the NRC's revised definition of byproduct material for events with an event date >= 11/30/2007.

Event Date The date the event occurred or began. The earliest date allowed is 1/1/1970. Only one date in the date range is required. Dates can be entered by typing a valid date (formats include mm/dd/yyyy and mm/dd/yy, with or without the "/") or by clicking the calendar icon. (Optional)
State The State of Record for the Licensee/Reporting Party. (Optional)
Site of Event The location where the event occurred. (Optional)
NRC Region The NRC Region for either the Licensee/Reporting Party or the Additional Involved Party. Select which party to include and then select the NRC Region (1-4), HQ (Headquarters), NR (Not Reported), NA (Not Applicable), Foreign or All. Note that all material licensees for Region 2 were transferred to Region 1 effective 10/1/2003.
Regulated By Select “Agreement State” if the event is regulated by an Agreement State, “NRC” if the event is regulated by the NRC, "Foreign" if the event is regulated by a foreign entity, or “All”. (Optional)
IAEA Category The IAEA category for an individual source (Sources Only), the IAEA aggregate category for all sources involved in an event (Aggregate Only), or both (All). The categories correspond to the relative hazard, with Category 1 being the most hazardous.

For Sources Only, NA (Not Applicable) means that the radionuclide is not one of the 16 radionuclides included in the IAEA guidance (regardless of the activity). NR (Not Reported) means that the IAEA Category could not be determined because either the radionuclide is not known, or the radionuclide is one of the 16 radionuclides in the IAEA guidance but the activity is not known.

For Aggregate Only, NA means that none of the radionuclides involved are included in the IAEA guidance (regardless of the activity). NR means that at least one of the sources involved had an IAEA category of NR (see above). (Optional)

Not Closed - A record is “Closed” when there is no further information expected regarding the event (i.e., no further inspections or reports, all information leads have been exhausted). This determination is made by the responsible regulatory agency. Note that this field was added on 11/1/2002. Previous records have been marked as closed only if the INL NMED staff received specific direction from the responsible regulatory agency.

Not Complete - A “Complete” record contains the minimum information deemed necessary to adequately describe an event. NRC's procedure "Reporting Material Events" (SA-300) specifies what information is required for a complete record. The complete record determination is NOT valid for records created before 1/1/1999 (item numbers < 990001). Note that records with item numbers >= 990001 may have event dates earlier than 1/1/1999.

Recently Updated - Event records created or most recently modified since the specified date.

Update Date The date the event record was created or most recently modified, whichever is greater. The earliest date allowed is 1/1/1970. This field changes every time a record is modified such that only the most recent modification date is stored. Dates can be entered by typing a valid date (formats include mm/dd/yyyy and mm/dd/yy, with or without the "/") or by clicking the calendar icon.
State The State of Record for the Licensee/Reporting Party, or the State of Record for the Additional Involved Party, or the Site of Event. (Optional)
NRC Region The NRC Region for either the Licensee/Reporting Party or the Additional Involved Party. Select which party to include and then select the NRC Region (1-4), HQ (Headquarters), NR (Not Reported), NA (Not Applicable), Foreign or All. Note that all material licensees for Region 2 were transferred to Region 1 effective 10/1/2003.

Reference Document Number - Each NMED record was developed from specific reference documents, such as Event Notifications, Preliminary Notifications, Inspection Reports, and Agreement State Event Reports.

Reference Document Number The documents used to create an event record. Enter a value and click "Find" to create a pick list. Entering xyz returns a pick list with report numbers beginning with xyz. Entering *xyz* returns a pick list with report numbers containing xyz. Select a specific report from the pick list; otherwise, the entire pick list is used.

Typical formats include: ENXXXXX for NRC Event Notification Reports, PNRYYXXX for NRC Preliminary Notification of Occurrence Reports (where R is the NRC Region, YY is the two-digit year, and XXX is the incremental value), MLXXXXXXXXX for ADAMS documents, DOTXXXXXXXXXX (where XXXXXXXXXX is the report's barcode number) for DOT Hazardous Materials Incident Reports, and ZZAAAAAA for Agreement State Event Reports (where ZZ is the state abbreviation and AAAAAA is the Agreement State Item Number).

Serial Number - Device (such as a moisture/density gauge) and source Serial Numbers may be included in NMED records.

Serial Number The manufacturer’s serial number for the device/associated equipment or the source of radiation. Enter a value and click "Find" to create a pick list. Entering xyz returns a pick list with serial numbers beginning with xyz. Entering *xyz* returns a pick list with serial numbers containing xyz. Select a specific serial number from the pick list; otherwise, the entire pick list is used. Try various serial number formats (spaces, dashes, etc.) to ensure an accurate search.

ADVANCED SEARCHES

Users may develop (and save for future use) Advanced Searches to meet their specific needs. The fields available for searching are grouped into categories that are similar to the various screens of the NMED data entry software. Clicking the “+” button next to a category will display the applicable searchable fields. Fields from different categories can be combined into a single search. The categories are:

General Event Information
Field Name Help Text (Displayed when clicking on field label)
Item Number The unique six digit item number. The first two digits represent the year in which the event was entered into NMED. Event Type Click on an event type to see its definition. Events may be categorized by more than one event type. If more than one box is checked, verify the setting of the AND/OR radio button.
Event Date The date the event occurred or began. The earliest date allowed is 1/1/1970. Only one date in the date range is required. Dates can be entered by typing a valid date (formats include mm/dd/yyyy and mm/dd/yy, with or without the "/") or by clicking the calendar icon.
Discovery Date The date the event was discovered. The earliest date allowed is 1/1/1970. Only one date in the date range is required. Dates can be entered by typing a valid date (formats include mm/dd/yyyy and mm/dd/yy, with or without the "/") or by clicking the calendar icon.
Date Reported to Agreement State The date that the event was initially reported to the Agreement State. This field is not used for NRC-regulated events. The earliest date allowed is 1/1/1970. Only one date in the range is required. Dates can be entered by typing in a valid date (formats include mm/dd/yyyy and mm/dd/yy, with or without the "/") or by clicking the calendar icon.
Date Reported to NRC The date that the event was initially reported to the NRC. The earliest date allowed is 1/1/1970. This field is complete for all NRC-regulated events. For Agreement State-regulated events, this field is complete for events reported since 1/1/2003 and is partially complete for older events. For events requiring notification to the NRC Operations Center, the date on the Event Notification Report takes precedence over other notification dates. Only one date in the range is required. Dates can be entered by typing in a valid date (formats include mm/dd/yyyy and mm/dd/yy, with or without the "/") or by clicking the calendar icon.
Update Date The date the event record was created or most recently modified, whichever is greater. The earliest date allowed is 1/1/1970. Select a specific party from the pick list; otherwise, the entire pick list is used.
License # The license number for either the Licensee/Reporting Party or the Additional Involved Party. Select which party to include and then enter a value and click "Find" to create a pick list. Entering xyz returns a pick list with license numbers beginning with xyz. Entering *xyz* returns a pick list with license numbers containing xyz. Select a specific license number from the pick list; otherwise, the entire pick list is used.
NRC Docket Number The NRC docket number for either the Licensee/Reporting Party or the Additional Involved Party. Select which party to include and then enter a value. Agreement State licensees do not have an NRC docket number.
NRC Program Code The NRC program code for either the Licensee/Reporting Party or the Additional Involved Party. Select which party to include and then enter a value. Agreement State licensees do not have an NRC program code.
State The State of Record for either the Licensee/Reporting Party or the Additional Involved Party. Select which party to include and a state. Select a specific city from the pick list; otherwise, all cities will be used.
Site of Event The location where the event occurred. Select a state or foreign country. Select a specific site from the pick list; otherwise, all sites will be used.
Narrative Search A summary of the event. Enter the word(s) to be searched for. Select the "All words (And)" radio button to return event records containing all of the words entered. Select the "Any words (Or)" radio button to return event records containing any of the words entered. Use double quotes to specify a phrase (like "medical facility"). This search does not accept wildcard symbols (like * and ?).
NRC Region The NRC Region for either the Licensee/Reporting Party or the Additional Involved Party. Select which party to include and then select the NRC Region (1-4), HQ (Headquarters), NR (Not Reported), NA (Not Applicable), Foreign or All. Note that all material licensees for Region 2 were transferred to Region 1 effective 10/1/2003.
Regulated By Select “Agreement State” if the event is regulated by an Agreement State, “NRC” if the event is regulated by the NRC, "Foreign" if the event is regulated by a foreign entity, or “All”.
NRC Reportable Select “Yes” for events that are reportable per NRC regulations, “No” for events that are not reportable per NRC regulations, “Uncertain” for events in which the NRC reportability has not been determined, or “All”.
Agreement State Reportable Select “Yes” for events that are reportable per Agreement State regulations, “No” for events that are not reportable per Agreement State regulations, “Uncertain” for events in which the Agreement State reportability has not been determined, or “All”. NRC-regulated events have this field marked "No".
AEA Material Select “Yes” for events that involved AEA material, “No” for events that do not involve AEA material, “Uncertain” for events in which the AEA material designation has not been determined, or “All”. Events that involve both AEA material and Non-AEA material are marked "Yes". Note that NMED implemented the NRC's revised the definition of byproduct material for events with an event date >= 11/30/2007.
Abnormal Occurrence Select “Yes” for events classified as abnormal occurrences (AOs) in the Report to Congress on AOs (NUREG-0090), “No” for events that are not AOs, “Potential” for events that are being considered for inclusion in the next Report to Congress on AOs, or “All”.
Investigation Select "Yes" if the NRC or Agreement State performed an investigation or "No" if an investigation was not performed, or "All".
Record Complete Select "Yes" for events considered complete per SA-300, "No (includes Info Request Pending)" for events that are missing information (including events for which an Information Request is pending), "Info Request Pending" for events in which a request for additional information has been sent from the INL to the applicable regulatory agency with a response pending, or "All". The complete record determination is NOT valid for records created before 1/1/1999 (item numbers < 990001). Note that records with item numbers >= 990001 may have event dates earlier than 1/1/1999.
Event Closed by Region/State Select “Yes” for events that are considered closed by the applicable regulatory agency, “No” for events that are not closed, or “All”. Note that all events are designated as not closed until a specific request to close the event is received.
Cause/Corrective Action/Keyword Fields

Cause The event cause category and/or sub-category. Select a specific sub-category from the pick list; otherwise all sub-categories will be used.
Corrective Action The action taken by the licensee to prevent recurrence of the event.
Keyword Keywords are added to events to allow grouping them during data searches. Note that this field only searches the Keyword field. To perform a search in the event narrative, use the Narrative search under General Event Information.
Device/Source Details Fields

Device The device/associated equipment.
Source The source of radiation.
Manufacturer The manufacturer of either the device/associated equipment or the source of radiation.
Model Number The manufacturer’s model number for the device/associated equipment or the source of radiation. Enter a value and click "Find" to create a pick list. Entering xyz returns a pick list with model numbers beginning with xyz. Entering *xyz* returns a pick list with model numbers containing xyz. Select a specific model number from the pick list; otherwise, the entire pick list is used. Try various serial number formats (spaces, dashes, etc.) to ensure an accurate search.

Medical Event Details Fields

Number of Patients The number of patients involved in the event. Only one value is required.
Therapeutic Procedure The therapeutic procedure performed.
Organ The target organ or target site of a therapeutic procedure.
Dose The therapeutic radiation dose, in rad, received by the patient.
% Overexposed The percentage that the administered dose exceeds the prescribed dose. Only one value is required.
% Underexposed The percentage that the administered dose is less than the prescribed dose. Only one value is required.
Diagnostic Study The diagnostic study performed.
RadioPharmaceutical The pharmaceutical or chemical name of the radiopharmaceutical or sodium iodide administered.
Personnel Overexposure Details Fields

Number of People The number of people involved in a non-medical radiation overexposure event. Only one value is required.
Dose The dose, in rem, received during a radiation overexposure event (non-medical). Only one value is required.
Type of Exposure The type of non-medical radiation overexposure.
Release/Contamination Details Fields

Type of Release/Contamination The type of material release or contamination that occurred.
Radionuclide The radionuclide involved in the event.
Activity The activity of the radionuclide, in Curies. Only one value is required.

Reporting Requirements Fields

10 CFR The applicable NRC reporting requirement number from 10 CFR. Enter a value and click "Find" to create a pick list. Entering 20 returns a pick list with reporting requirements beginning with 20, such as 10 CFR 20.2201. Select the specific reporting requirement, or the selection at the top which finds all that 'start with' the input value.

EXAMPLES OF ADVANCED SEARCHES


    Overexposure events that occurred from January 1996 to present that are reportable per NRC regulations
    All Agreement State regulated events that occurred in Texas in 2001
    All events that involved both an Equipment Problem AND a Personnel Overexposure
    All events that involved a device or source with a serial number containing 254
    All events reported under requirement 10 CFR 20.2201(a)(1)(i) that occurred in 2003


Overexposure events that occurred from January 1996 to present that are reportable per NRC regulations.

1. Click the "Advanced Searches" tab.
2. Click the + button next to "General Event Information".
3. Next to the "Event Type" field, click the box next to "Overexposure".
4. Next to the "Event Date" field, enter 010196 in the ">=" field.
5. Next to the "NRC Reportable" field, click the "Yes" radio button.
6. Click the "Search" button.

All Agreement State regulated events that occurred in Texas in 2001.

1. Click the “Advanced Searches” tab.
2. Click the “+” button next to “General Event Information”.
3. Next to the “Event Date” field, enter 01/01/2001 in the “>=” field and 12/31/2001 in the “<=” field.
4. Next to the “Site of Event” field, select “Texas” from the pick list.
5. Next to the “Regulated By” field, click the “Agreement State” radio button.
6. Click the "Search" button.

All events that have both an Equipment Problem AND a Personnel Overexposure associated with them.

1. Click the “Advanced Searches” tab.
2. Click the “+” button next to “General Event Information”.
3. Next to the “Event Type” field, click the “Equipment” and “Overexposure” boxes and the “And” radio button.
4. Click the “Search” button.

All events that involved a device or source with a serial number containing 254.

This search can be performed using Basic Searches or Advanced Searches. To execute this search as a Basic Search, perform the following steps:

1. Click the “Basic Searches” tab.
2. From the “Select a Search” pick list, select “Serial Number”.
3. Next to the “Serial Number” field, enter *254*.
4. Click the “Search” button.

To execute this search as an Advanced Search, perform the following steps:

1. Click the “Advanced Searches” tab.
2. Click the “+” button next to “Device/Source Details”.
3. Next to the “Serial Number” field, enter *254*.
4. Click the “Search” button.

All events that were reported through a Preliminary Notification from Region III in 2002.

This search can be performed using Basic Searches or Advanced Searches. To execute this search as a Basic Search, perform the following steps:

1. Click the “Basic Searches” tab.
2. From the “Select a Search” pick list, select “Reference Document Number”.
3. Next to the “Reference Document Number” field, enter PN302.
4. Click the “Search” button.

To execute this search as an Advanced Search, perform the following steps.

1. Click the “Advanced Searches” tab.
2. Click the “+” button next to “Reference Documents”.
3. Next to the “Reference Document Number” field, enter PN302.
4. Click the “Search” button.

All events reported under requirement 10 CFR 20.2201(a)(1)(i) that occurred in 2003.

1. Click the “Advanced Searches” tab.
2. Click the “+” button next to “General Event Information”.
3. Next to the “Event Date” field, enter 01/01/2003 in the “>=” field and 12/31/2003 in the “<=” field.
4. Click the “+” button next to “Reporting Requirements”.
5. Next to the “10 CFR” field, enter 20.2201 in the text box and click the “Find” button.
6. On the pop-up screen, click the “20.2201(a)(1)(i)” box.
7. Click the “Search” button.
